Faculty & Staff Insurance Registration

The enrollment buttons below are for faculty and staff engaging in uiniversity-related activities or travel abroad. After July 31, 2019, coverage will be provided to all faculty and staff free of charge who enroll.

About the Application

The insurance application cannot be saved. Registrants should have all information in advance. Please refer to the list below for the information you will be asked to supply when enrolling.

Graduate students participating in university-related travel must enroll in international insurance as students. Units can still pay for student insurance by entering CFOP information in the billing questionnaire. Go to the Student Insurance Registration page.

  • Location(s) of travel
  • Date of birth
  • Gender
  • UIN
  • Passport number and expiration date
  • Upload a scanned copy of your passport
  • Upload a scanned copy of the "EU GDPR Acknowledgement and Consent Form"
  • In-country address and phone number
  • Emergency contact name, address, phone number, and relationship to registrant

EU GDPR Acknowledgement & Consent

If you are traveling to a country in the European Union (EU) or the European Economic Area (EEA), you must download and sign the EU GDPR Acknowledgement and Consent Form (PDF).

  • European Union countries: Austria, Belgium, Bulgaria, Croatia, Republic of Cyprus, Czech Republic, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Ireland, Italy, Latvia, Lithuania, Luxembourg, Malta, Netherlands, Poland, Portugal, Romania, Slovakia, Slovenia, Spain, Sweden, and the United Kingdom.
  • European Economic Area countries: EU countries (above) plus Iceland, Liechtenstein, Norway, and Switzerland.

Enrollment by Proxy

To enroll faculty/staff by proxy, start by logging in with your own NetID and password. After you have logged in, enter the traveler’s university email address in the “Keywords” section and change the “Directory to Search” from SIS to HR before selecting “Search.” Under the Action column in the search results section, select “Register” for the traveler and proceed following the same enrollment guidelines listed above.

After you Enroll

Travelers will receive immediate confirmation of their enrollment via email from the university. The university will process the enrollment and travelers will receive an email from "accouncements@gallagherstudents.com" with necessary information within five business days.
